The coalition members meet and strategize approaches to making Stanislaus
County an “asthma friendly” community by promoting awareness, education,
management, and prevention. General meetings will be held quarterly on
Wednesdays in the Martin Conference Room.

The 2010 - 2020 Framework for a Thriving Stanislaus is a plan created by a large group of individuals and public and private organizations to coordinate community efforts to improve the health of our county and well being of our citizens. The Framework for a Thriving Stanislaus is a living plan, which means it is constantly being updated as needed. We encourage you to return to this page to see how the plan develops as we make progress on achieving the goals.
The Framework for a Thriving Stanislaus is organized into four areas that impact community prosperity and well-being, also known as broad determinants of health, which are social, economic, infrastructural and environmental factors that influence health, well-being and prosperity. These are:

The HEART Coalition was convened in 1999 due to the high
rates of heart disease in Stanislaus County. The coalition’s
mission is to reduce cardiovascular disease risk factors to
improve the health status and quality of life of the residents
of Stanislaus County. The Coalition has sponsored several
projects in an effort to fight the obesity epidemic and reduce

The Hepatitis C Task Force is representeded by members of
the HIV/STD program, Heath Service Agency Epidemiologist,
Public Health Officer, Behavior Health and Recovery Services
Alcohol and Drug Program, Veterans Services, Jail Medical
(CFMGJ), Kaiser, Communicable Disease Program, Golden Valley
Homeless Program and non-profit agencies. The primary task
of this group is to develop a Hepatitis C plan for Stanislaus
County.

The primary task of the Local Implementation Group is to develop a
comprehensive HIV plan that is based on scientific evidence and
community values and is responsive to the local epidemic.
The group meets 3rd Wed of every month from 12-1.

A group of private and public organizations, agencies, business leaders
and concerned citizens working together to protect infants and young
children against vaccine preventable diseases. DMC Foundation is the lead
agency. Meetings are held monthly, the 4th Thursday 12 noon at the
Stanislaus County Health Services Agency, Martin Conference Room.

A multidisciplinary group that shares the goal of assisting women to have
healthy, full term infant births. The group meets regularly and has developed
strategies to reduce the incidence of low birth weight babies born in our county.

This coalition is made up of a variety of organizations and
individuals that come together to discuss, collaborate, and work
on projects that are related to the promotion of healthy eating
and physical activity. Meetings are held quarterly from 3:00-4:30 in the Martin Conference Room.

The purpose of the committee is to assist in developing and reviewing
the dental project application for funding, and to implement yearly
projects to increase dental disease prevention awareness for children
in the county. Meetings are held quarterly in the Public Health Conference Room from 5:30-7 p.m.

A workforce and information sharing meeting exclusively for those
who work in promotora or community health worker capacity.
These meetings are held in Spanish only and usually take place
the last Wednesday of the month from 10-12.

Dedicated to Making Stanislaus County Safer for All
The Safe Communities Coalition of Stanislaus County aims to decrease
the rate of injury and death to child passengers, bicyclists and
pedestrians. The Coalition works to empower and mobilize Stanislaus
County residents, by supporting community awareness and education that
creates safe, walkable and healthy communities. Current Coalition
activities include coordinating Walk to School Day events at select
schools and the Walking School Bus program. Meetings are held quarterly.

The Teen Pregnancy Prevention Collaborative is comprised of various community
members and organizations. Organizations consists of Stanislaus County Office of
Education, local Family Resource Centers, local school districts, Project YES,
Ceres Partnership for Healthy Children, Planned Parenthood, and SHADE Teen clinic.
The Collaborative mission statement is: "To promote health relationships,
sexuality, and lifestyles among teenagers and young adults in Stanislaus County".
The goal of the Collaborative is: "To ensure that youth in Stanislaus County receive
comprehensive sex education and have easy access to confidential free or low cost family planning services".
The Collaborative meets quarterly (March, June, Sept., & Dec.) on the first Wednesday
of the month from 3:00pm - 4:30pm. Please call for meeting locations. The Collaborative
welcomes any member of the community to participate and attend meetings.
